Doubtful that this form will hold up in court.
It lacks the fields of a phone number and a email address which usually needed when doing street photography...
I would love to have a correctly written form that was carbonless to give a copy to the model at the time the agreement is executed.

VERY UNPROFESSIONAL LOOKING CARDS. What a disappointment! Definitly not what I was expecting. The cards are very small and the actual release is so small you need a magnifying glass to read it. Use these cards in a pinch only after you discovered you left your real cards at home. MY ADVICE, DOWNLOAD A GOOD PDF AND USE THE MONEY TO HAVE THEM PRINTED AT YOU LOCAL COPIER,PRINTER

These model releases are PERFECT! They are small and compact, allowing me to easily store them in my camera gear bag without taking much room at all. The only complaint I have (due to my OCD) is that the binding isn't the best. I ordered two packs and on both packs some of the cards were not bound to the glue and fell out, but that is not going to stop me from purchasing more! I personally think these releases are the best model release products out there at this time.

Small size means I am likely to have it with me when needed. Simple wording lets suject know what they are signing.
Great to have for just incase when you originally did not intend to publish but plans changed.

These are small, about the size of an index card, so they slide easily into a pocket. Still, they include all the info that you need for a release card and take all the guesswork out of creating your own.
Buy a pack and throw a few into each bag that you carry so you will always have some on you. Throw some into your glove compartment. Then when the situation arises you will be prepared.

I like too shoot photos and I like too enter photo contests. Sometimes I shoot photos of family, friends and other people I know. Some photo contests require a model release form when a person is in the photo. These release cards are simple and easy too understand.
