Max/MSP from Cycling 74 is a set of software visual tools and objects, presented in a graphical programming environment, which allows users to create their own software. The various modules are connected to each other through virtual patch cords.
The basic environment that includes MIDI, control, user interface and timing objects is found within Max. Built on top of Max are hundreds of audio processing objects, included in MSP that allow for creating everything from interactive filter design to complete hard disc recording.
Jitter extends the Max/MSP programming environment to support real-time manipulation of video, 3D graphics and other data sets within a unified processing architecture. Because Jitter, like Max/MSP, is generic in nature, it offers unlimited possibilities for creative exploration, whether in video processing, interactive art, teaching new media, or data visualization.

I really love this program - but it's a fairly steep learning curve. It's an extremely open and powerful means of manipulating audio and video. But they've recently revoked their support of Pluggo and concentrated their support on Abelton Live integration. That and the buggy DRM in OSX Lion were some small tarnish on an otherwise life-changing piece of software.

Interactivity is the new medium. MAx MSP and Jitter opens the door to interactive programing for artists. It allows you to create your own effects, built your own software instruments and develop interactive music and video projects...and that's just the beginning.
Its a very sophisticated program and will require some effort. but the basic premise is you can process any information and turn it into an action.
I invested a lot of time researching my options. I found other programs that have similarities to what you can do with MAX. What sold me was MAX's visual interface and seeing that artists were pushing the envelope of what is possible.
