For the configuration of connecting "1/4" TRS in to XRL out", what is the pinout for the XRL connector? Does it use TRS L or R?

No, the Kopul HMX-2 Humbuster - Dual-Channel Hum Eliminator with XLR and 1/4" Connectors is a passive device that does not pass phantom power. It is not intended for microphone use, it is to be used with instrument or line level devices.

This is not a powered unit, it's completely passive and only connects to your audio feeds, so the frequency of your AC power is irrelevant. In my opinion the information about this unit should read "eliminates unwanted hum and buzz caused by ground loops".

Is there a version of this device that can eliminate hum generated by a turntable ( Pro-ject turntable connected via 2 heavy duty shielded RCA cables to a Denon AVR-X4200W integrated amp; I have tried a number of possible fixes to no avail). Or could this device be used for this purpose with some adaptation?

B&H sells adapters and cables like GPR101 or CPR202 that will let you connect your turntable to the Humbuster to the amp. You may also consider picking up 3-to-2 prong grounding adapters to see if that works. There are some turntables that have inherent hum due to build quality or wiring. I hope you get to enjoy your vinyl again soon!
