The Monopod Head with Standard Screw-Knob from Really Right Stuff is designed for use with monopods. The head tilts a full 180 degrees. The indexed quick release clamp is oriented for vertical fore and aft, 180-degree lens tilting but you can change the orientation to horizontal, 180-degree camera tilting by turning the clamp 90 degrees using the supplied hex key. Portrait orientation is achieved with L-plates or rotating lens collars. The monopod head will support up to 75 lb, far in excess of what's needed for any type of image making. It has an Arca-type compatible dovetail base for mounting on Really Right Stuff heads or accessories. It also accepts separately purchased Arca-type camera or lens plates from Really Right Stuff, Wimberley, Kirk, Markins, Acratech and others.

Will this fit the Canon 400 mm 2.8 Version II with the Canon foot or do you need to replace it?

The standard Canon foot would not work unless you have a swiss arca plate to screw it on to.I have shot with that lens. The unit will handle the weight, but if you loosen the big screw, be careful of the weight. For primes that size and larger, I would feel more comfortable with a gimbal tripod head.
