This bag is very well constructed and designed (for example, the zippers are first-rate). I use it for a RRS TFC-24L tripod with a BH-40 ball head attached. The combination fits well, but there certainly isn't much room to spare.

Does the 4.7" width refer to the base or top of the bag? Trying to find a bag that will fit a Sirui W2204 with Benro GD3WH head.

The Really Right Stuff Medium Tripod Bag's (Black) 4.7" diameter is the measurement at the top of the bag where it is widest. The Sirui W-2204 Waterproof Carbon Fiber Tripod (BH #SISUW2204) paired with the Benro GD3WH 3-Way Geared Head (BH #BEGD3WH) will fit in the Really Right Stuff Medium Tripod Bag (Black).

Would the rock claw feet chew up the bottom of this bag?

Yes I do believe it would over an extended period of time. It would be easy enough to place a piece of hard plastic in the bottom of the bag to prevent any abrasions or you could simply remove the feet. I would not see this as an issue and am happy with mine. I have the claw feet and the spike feet for my tripods.
