No, a carrying case is not included with the LH-302 headset. The compatible Telex LH-SC RTS Carrying Case for LH Series Headsets (B&H # TELHSC) is available for purchase separately.

Does the 5-pin male LH-302 separate channels between ears like my PH44? Channel 1 in one ear and Channel 2 in the other ear?

The internal wiring for the Telex LH-302 Lightweight RTS Double-Sided Broadcast Headset and the Telex PH-44 Lightweight Dual-Sided Headset is the same.

Can the mic be muted, or does that require a belt pack?

The Telex LH-302 Lightweight RTS Double-Sided Broadcast Headset does not have an onboard mute switch for the mic. The microphone will need to be muted externally, from a switch on the beltpack or device that the headset is connected to.
