What is the power of the included POE injector ...
What is the power of the included POE injector like is it BT or AF?
When using a 60W injector on the USW-Flex, there is a setting that has to be set in the device properties in the Ubiquiti controller tool to tell the switch that is can use 60W. Otherwise it will assume it cant use more than 25W, and you'll see devices power up inconsistently. This is very poorly documented and many posts on the internet have outdated screenshots of the controller tool.
