You may use either the Cokin Z-PRO Filter Holder, or the Cokin BPW400 Wide Angle Filter Holder for P Series, B&H # COWAFHP, for use with lenses wider than 24mm.

This Cokin P series filter holder does not come with an adapter ring. You would need to purchase a 52mm adapter ring (B&H# COARP52) to use this filter holder on a lens that has a 52mm filter thread.

I am trying to figure out how to add a polarizer to it. Do I need a special polarizer or additional adapter ring?

Cokin makes a polarizer for this that fits in the slot closest to the camera lens and behind the slots for the ND filters. It works, but is a bit stiff to turn.

how do I attach the holder to my camera?

The holder simply replaces the lens cap when in use. It just screws onto the same way the lens cap does. Obviously it will fit only on cameras with removable type lenses such as DSLRs, not on the typical pocket or point-and-shoot camera.
