Would this work with the LG GX series OLEDs? In other words, is it completely flush and would allow for the tv to be completely flat against the wall?

Does this unit come withe the flange and cover kit as shown in the picture, or only the box?
The Flange and Cover are not included. Instead you would need B&H # CHPAC525FC which is the same box with the Flange and Cover included.
