Does this bodypack come with the mic capsule or just the bodypack? Also, does BH sell a replacement mic capsule for this bodypack? We have a broken capsule that we will need to replace.

The Shure QLXD1 Digital Wireless Bodypack Transmitter (BH #SHQLXD1G50) does not include a microphone element with purchase. There is a vast selection of compatible microphones that can be used with the QLXD1 for various applications. The Shure WL185 Cardioid Lavalier Microphone with TA4F Connector (BH #SHWL185) is a common option: https://bhpho.to/3NQusKe.
