The description says there are 3 holes 4.25" apart. There are only 2 holes in the picture. Can someone please tell me how many holes and how far apart they are, actually?
This page has three Holes spaced 4.25" apart. The photo does show 3 holes but you have to enlarge the photo and look closely.

Is it self adhesive? What are the dimensions?
No, the clear plastic is sealed at the sides and bottom. You feed the photo in from the top and it is held in place by friction with the paper divider.
