after a year and a half of owning this - I do love having a grip and have no issues with the grip itself however the rubber on each side has started to unglue and is disappointing to see it fail within a year and a half. Nikon has informed me that it would cost almost 200$ to get this fixed. Photo added for when it initially started to unglue.

Is battery changing the same as on the previous Z grip, or does it use a "slide out" holder as with the D850 grip? Will it accept the larger EN-EL18 or only two small batteries?
The grip has a removable tray that fits two EL15 batteries and loads in from the end, and locks into place. The end cap is hinged like the MBN-10 and closes the tray and batteries in. The left-side battery, like the MBN-10, drains first, and can be swapped without removing the tray (there is a latch to hold the tray into the grip, and a latch on the tray itself to retain that battery).

Will it function with only 1 battery installed? I know it's hot swappable, and drains the A battery 1st. Can I pull the A battery to charge it while only shooting with the B battery installed?

You can only hot-swap the A battery while the camera is still on. There's a separate lever to remove only the A battery instead of both batteries at the same time.

do you have to remove the door of the battery compartment on the Z6ii in order to use this grip? if so, is that difficult to do
To use the Nikon MB-N11 Power Battery Pack with Vertical Grip the battery door on the camera must be removed. There is a spot on the battery grip to store the door for future non gripped use.

Does the MB-N11 battery grip's function button, wheel and joystick have programability within the Z6 II camera's menu system like the camera's wheel, joystick and buttons? Or, are the MB-N11's functions set/fixed by Nikon?
