This Baader Planetarium Mounting Block from Alpine Astronomical is a replacement or spare for the one included in the Baader Leveling Weight Set for PlaneWave OTAs. It accepts the M14-threaded counterweight bar and comes with two 1/4"-20G screws, 30mm long.
