Can I output to RCA with an adapter with no issues?
No you can not use RCAs with phantom power. RCA cables are not balanced.

The ART Phantom I single-channel phantom power supply requires an AC adapter for use. The adapter is included in the packaging contents.

Does the power supply only travel in one direction? ie along the input path so it travels only to the mic?
Yes, the power goes to the mic only.

Does this add hiss or ground-loop hum to the signal or is it clean?
I installed the Art phantom power supply in our pulpit microphone circuit when the phantom power supply portion of our sanctuary mixer board would intermittently fail. I have not noticed any unusual hiss or hum from the sanctuary speakers or our Youtube live stream recording. I cannot say how it would do for critical recording. The ART power supply does have internal isolation capacitors and a small external AC power supply (wall wart) which will provide a little more isolation from ground loops.
