I am looking for something like this but with a baby plate to mount to the ceiling instead of mounting two parts with a separate baby plate. Do you sell any telescopic extensions with baby plate?

If looking to bolt the plate into the ceiling, you may only do so by using a separate baby plate like the Matthews Baby Plate - 3 B&H # MABP3 with this extension.

How much weight could this hold if i decide to use this as a boom arm?

This extension is not recommended for use as a boom arm, therefore the manufacturer would not offer a load capacity in this case. Matthews does offer the Matthews Baby Boom, B&H # MABBM which can fit light stands with a 5/8" pin, specifically C-stands or roller stands. https://bhpho.to/2ZCoRhS

Why is it called baby ? - makes it sound like it's smaller than a standard light stand - will it only work with baby stands?

Baby refers to the size of the mounting post atop the stand and not the size of the stand. It's the most common size used on photographic light stands. The next size up is junior which is quite a bit larger and made for heavy light fixtures. So yes, this extension will only attach to a stand with a baby post.
