I am looking to mount this into a rotatable tray for a matte box tray. Is the diameter of this unmounted filter exactly 4.5"? It seems the stated diameter of the mounted variety is also 4.5"

Yes you are correct. Square or rectangular filters are attached to lenses via filter holders, most of which rotate the filter independently of the lens.
