How is this used? I have a Metz 5512 bracket attached to a Metz 60CT-1 and an RC0 hex plate in the 3/8 socket on the bracket, but cannot tell how this works.

The idea behind this screw is to replace the screw on your RC0 plate, and allow it to go through the plate and through the bracket to screw directly into your camera. So then you have a bracket with an RC0 plate attached to it.
