How far does it extend foward from the flash head?

i have not measured but i would guess about 8 inches. not sure why you ask. i use it on a light stand. it would be a little bulky if you plan to use it on camera. hope this helps.

Does it have any sort of frame inside that keeps it in shape or does it collapse into itself? Does it diminish the power output of the flash a lot?

It has no frame, so you can just roll it up. And it does diminish the output. it also has a tendency to pull itself off of the flash by it's own weight. It's a pretty decent accessory in my opinion, but make no mistake about it, It's pretty limited in almost every regard.
