The Movcam Twist Handle is a quick-release top handle designed for use with Movcam's GH5 and a7 II-series camera cages, attaching to the cage via an included short NATO rail. Once attached, the handle can be rotated by loosening a top thumbscrew, letting you set the desired grip angle/position. Tightening the screw locks the handle in place. For accessory mounting, the handle features 1/4"-20 and 3/8"-16 threaded mounting holes, a single 15mm rod port, and two shoe mounts (one in front and one in back of handle).

Is this Top Handle compatible with NATO standard rails?
While the clamp is NATO shaped, it appears too wide. I would have to go with what Movcam says and that is this requires the Top Plate (sold separately) for mounting.

I'd like to use this with a Viewfactor BMPCC cage. But sometimes the mounting holes are different spacing (proprietary). I found this out with the Wood Camera handle, although they do supply an additional adapter to allow various widths. Will this handle work with othe cages or is it just for the Movcam cage?
I would say that it is more than likely the Movcam Top Handle will fit the Viewfactor as well as others.

Yes, not a problem. The camera has several screw hole mounting points on top just for this purpose.

This Movcam top handle does not come with a small NATO rail. This is made to attach to rigs that already have an existing NATO rail.

Maybe a silly question, but how do you 'twist' the handle? I've tried removing the big screw in the middle, but that doesn't work.
The handle doesn't exactly twist. It's fixed. You just grab it and hold the camera with it.
