I want to capture the sound on a phone, the audio signal is coming from an HDMI source going through an HDMI audio extractor, will this cable do the correct job of mixing left and right channels?
The Sescom Stereo RCA to 3.5mm TRRS Plug Line to Mic Level Summing Cable will take the stereo output signal and combine the left and right channels to mono so the audio on both channels will be exactly the same.

Will this work to connect the stereo line out from an amp to a single channel 3.5 mm input of a subwoofer? (A simple y-splitter cable makes the amp sound horrible.)
No. This Sescom cable is a line-to-mic attenuating cable intended for connecting line level signals to the mic input on a mobile device like smartphone or tablet. The 3.5mm end is TRRS, not TRS. You can use a Y-cable such as the Hosa Technology REAN 3.5mm TRS to Dual RCA Pro Stereo Breakout Cable (B&H # HOYCRTRSRCA6) to connect the output of an amp to a subwoofer.

Will it work being connected to a Macbook Pro (2015) headphone jack? And for recording in a DAW?
Hi Lest. There is a chance it will work, but it most likely will not, unless the jack "sees" the signal in the same way it does an Apple earbuds set or a headset with mic. Our IPSUM and IPHONE cables were not designed for Macbooks, but we are reviewing and will be testing and updating the line accordingly once tests are complete and we can fully advise whether cables work with Macbooks or not.
