Does the head rotate so that the camera lens faces perpendicular to the ground? If so, would it hold the weight of a DSLR with a large lens and macro adapters?

yes, the head rotates which allows the camera to be positioned at various angles. The load capacity is 6 pounds, enough for holding a camera with a large lens.

Unfortunately, the 8" arm of the ALZO 1236 is not adjustable, but you will have access to alternate the ball head position.
