Am Sorry, I have never used a Olympus and hence can't give you a direct answer. However, if it helps in terms of comparison- it is more than adequate for Nikon D5 with a 24-70 f2.8 lens with hood.

Would the medium or large be better for a Sony a7R IVa with a Sony 24-105mm lens?
I use a medium for a GH5 and an EF 70-200 f/2.8 ii. Perfect fit. Feels a little big when used with my EF 24-70 f/2.8 ii. Guessing the Sony lens telescopes/extends so a medium will definitely fit. May be a little baggy though.

Depend on what kind of lens you intend to use with?
You should be fine so long as you don't use lenses bigger than typical 24-70 zoom lens.
I've used it with my nikon D5 and 24-70N(no VR)

You would be much better off using the Think Tank Photo Emergency Rain Cover (Large) for your needs.
