As the B&H rep has stated it is dual mono. So if you have a mono source into the SR-C2000, it goes to both left and right. And if you have a stereo source, it will be mixed into mono. If you need a 3.5mm mono or stereo cable, you can get the Saramonic LC-C35, but it does not provide plug-in power to microphones. So if you have a powered mic, output of a wireless mic receiver or a powered 3.5mm source, it will record onto your iOS device in mono or stereo, depending on the signal you feed it.

Yes, but you will not be able to hear the meeting or be able to select the phone's speakers to hear the meeting when using this, or any external Lightning microphone that does not have real-time monitoring functionality. This is a limitation of the Zoom mobile app.

Will this cord allow use of both ends of the V-Mic D4 Duo with an iPhone?

Yes, it will. But it will be a mono signal, so if the V-Mic D4 Duo has a stereo output (dual mono actually) with the front on the Left and back on the Right, it will mix them into a mono signal.

The Saramonic SR-C2000 3.5mm TRS Male to Lightning Adapter Cable for Audio to iPhone can be used to send mic level audio signal from a microphone or wireless microphone receiver to the iPhone, but the output signal from a mixer is line level, so you will need to use an attenuator to pad the signal and bring it down to mic level. You can combine the SR-C2000 with an attenuating cable like the Sescom 3.5 mm Line to Mic -25 dB Audio Cable for Zoom H6 (B&H # SELN2MICZMH6) or the Kopul ACH4-25MON Line-to-Mic Attenuator Cable (B&H # KOACH425MON).
