I have a case, which is open enough for charging my phone. Can I connect the mic without removing the case?
Although you are able to connect a Lightning cable to your phone with the case attached, it is not possible to say with certainty whether or not the mic fit while the case is attached. The Lightning connector on the mic is attached directly to the mic's body and the mic is designed to fit flush against the phone. You will have to test it to see if the case provides enough clearance around the Lightning port to accommodate the mic. The only way to know for sure is to try it out.

Could this be used vertically?
Yes, it could be used vertically but the kit was designed to be used horizontally for compatibility with the various accessories. The phone grip has three 1/4" mounting points so it can be mounted sideways to hold the phone vertically (portrait style). In doing so, you will be limited on options for mounting other accessories and may need to have the phone turned so that the mic is positioned on top.
