The F38 Quick Release Bottom Plate from Falcam integrates a standard quick release mounting plate to your gear configuration for efficient transitioning between mounting devices. How is this compatible with both a 3/8 and 1/4 inch tripod screw?
The Falcam F38 Quick Release Bottom Plate is compatible with both 1/4"-20 and 3/8"-16 tripods screws, by use of a 1/4"-20 to 3/8"-16 bushing that is included.

You have 2 extra holes in the first image, but none in the others. Can you clarify if your version has two extra holes or not?
There is only once version of the Falcam F38 Quick Release Bottom Plate, it will arrive as pictured in the 2nd and 3rd images. The first picture an older non-available iteration of the QR system.

F38 and Peak arca are not the same thing. This will not take an arca plate.
