If trying to set the white balance from a flash, ...

If trying to set the white balance from a flash, does this show a similar value to the flash temp (like around 5600K)?

The Manfrotto EzyBalance Gray Card is color neutral. If trying to get the correct white balance on a camera when using flash, this can give you the correct reading for a custom white balance. The reading itself is done in the camera or in software during post processing.

What is the purpose of autofocus reference lines ...

What is the purpose of autofocus reference lines on this card? If focusing is unnecessary to set the exposure, why are the reference lines printed on the card? Some gray cards do have autofocus reference lines, while others to do not. I can't understand this.

The autofocus lines are mainly to compose while your taking a reading. It makes more sense for a model to hold the target and to be in focus.

18% gray or 50%? I received my order today and next to a wallet size gray card I have, the difference is dramatic. 180+ reviews say it works fine, but I'm still interested to know if it's 18% or if I possibly received a different model.

I believe Manfrotto (Italy) actually owns the Lastolite brand, however the EZY Balance card is made in the UK.

I just received the Lastolite 20'' collapsible grey card. Is it normal that this thing stays warped and doesn't become completely flat once taken out of the pouch? If so, without a flat surface, you can't get an accurate reading so this thing is useless...

It's normal as it is collapsible, so when you unwrap, it cannot be completely flat. You may notice one thing the camera struggles to focus on EZYBalance Grey/White Card. The main purpose of white balance can be achieved whether the card is flat or not, just click on the card and shoot.

When should I use the grey side or the white side of a white balance card?

This depends a lot on how you use the card. I use it to shoot SLog3 video and only use the white side to set my exposure on the waveform monitor of 70-75%. This renders about the right exposure for Slog3. Never use the gray side. Just additional note: often use the white side fill reflector.
