I've always used Avenger (Manfrotto) products. The built quality is just top notch. The reason why I got buy the 20in vs the 40in Extension arm is for travel and portability. Great for mounting lights and grips, because of its length, It can support a bit more weight at the end of the rod without seeing any tension at the arm. This extension arm should be in most studio or on-location content creator's gear list. Good for almost anything, and can attach this to almost any non c-stand with a sand bag. Also you'll rarely or never see this item on third party markets. Will last you forever.

The grip head is fixed. In some cases and with other accessories you can use this for a overhead rig. You will want to make sure the particular stand has the load capacity for it.

is there a flat side at the point where you'd attach a light strobe to it when you put it at an awkward angle?
No flat side but not a problem, use it with a Grip Head and nothing will slip. I've hung several things on mine without any crashes.

The 40" arm load is limited by how much the stand it is mounted on can hold. You will want to make sure the attachments, counterweight and arm weight less than the stands load limit.
